The Texas Department of Insurance (TDI) is conducting an important survey that asks about the medical care you received for your work-related injury. The purpose of the study is to learn about your access to and satisfaction with the medical care you received as an injured worker. The information you provide is very important to these efforts to improve health care and other benefits for injured workers in Texas. This is your chance to help the agency serve you better. This study is funded by TDI and is being conducted by the Texas A&M University’s Public Policy Research Institute on behalf of TDI.

El Departamento de Seguros de Texas (TDI) está realizando una encuesta importante que le preguntara acerca de los cuidados médicos que recibió para su lesión relacionada con su trabajo. El propósito del estudio es para aprender acerca de su acceso a y su satisfacción con el cuidado médico que usted recibió como trabajador lesionado. La información que usted proporcionará es muy importante para nuestros esfuerzos de mejorar el cuidado médico y otros beneficios para los trabajadores lesionados en Texas. Ésta es su oportunidad de ayudar a la agencia a servirle mejor. The interview will take about 10 minutes to complete. You have been chosen at random to participate, your participation in this study is voluntary, and there are no negative consequences if you chose not to participate. If you choose not to, this will not affect the benefits you get. However, your input and opinions are very important to the success of the study. In addition, if you choose to participate, you may choose to skip some questions and still participate in the rest of the study.

Of course, what you have to say is confidential and such privacy is protected by the Texas Labor Code (§ 405.004. What you say will be used only by this study. Once the study is complete and the results are released, the survey results will be in summary format and no individuals will be identified.
